Patient Education Workflow

You are often the last person our patients see at SMLC.  

1. When patient is ready for check-out, click on the blue circled arrow to the right of patient’s name to begin the process.

2. Before moving further forward, please ask patient to state his/her DOB to confirm correct chart is pulled up.
3. Confirm patient photo is added to chart. If photo not yet added, please add photo (can scan from an ID or take a photo with an iPad or iPhone).
4. Under “RTO Orders” (RTO = return to office) will be indicated if/when provider would like patient to return for a follow-up visit. Please select “schedule” to set appointment.

1. 1. Select appropriate clinic and any provider, and then highlighted clinics will appear. Click on calendar date, and then you can select time below.
      1. Note: If scheduling a telehealth appointment, please schedule only Wednesdays and Fridays (except first Wednesday of the month) and only 1 telehealth appointment per time slot (max of 7 telehealth appointments per day). If telehealth is for a Spanish-speaking patient, please schedule both translator and patient together and include the translator’s name in the appointment note, so the provider knows who to call.

      2. For scheduling new patients (NP): Only schedule 4 NPs per clinic (when possible, at the following times (EST): 10:00, 10:30, 11:00, and 12:00). If patient expresses emergent situation and there are already 4 NPs scheduled at the next clinic, discuss possibilities with Medical Director or Nurse Manager

1. 2. After you schedule any needed follow-up appointment(s), click “go to appointment workflow” to continue check-out process.

5. The checkout screen shows the following which are taken care of by other staff members (not at patient education): Orders, Prescriptions, Labs, Imaging, Referrals.
6. If the patient is not set up with the Patient Portal, please discuss with patient if he/she would like to register. Portal invitations may be sent via email or the link found via the SMLC website (https://www.stmaryclinic.org/) via the Patient tab.
7. The Patient care summary (which summarizes his/her visit with SMLC and gives basic information for next steps in care) is available via paper (as below) or electronically (on the patient portal).
   1. If patient would like a paper copy, please print. If patient does not want a care summary, please select “decline.”

8. If recommended by provider, patient education materials will be indicated on this page for printing.
   1. Note: Even if not included, please ask patient if he/she has any new medications and ensure he/she understands how/when to take medication.

9. The schedule will turn grey when the patient is in the checkout process. The grey to the left of the patient photo (see below) means you still need to select “Done with Checkout.” After you do so, this grey portion will disappear. Great work!

1. In the upper left-hand corner, click “Calendar” and then select “Today’s Appointments.”
2. 
3. On the next page, select appropriate clinic and clinic date, and then click “Go.” After the clinic appears below, then select “Today.”
