Please greet our patients with a smile and a warm welcome!
When patient is ready for check-in, click on the blue, circled arrow to the right of the patient's name to begin. Note: If the patient is new to the clinic, “New Patient” paperwork should be completed.
If patient has appointment scheduled, skip this step. For drop-in visits, check if patient is established with SMLC by typing name in search box at the upper right-hand corner. If patient is not registered, click “Register.”
Before moving forward, please ask patient to state his/her DOB to confirm correct chart is pulled up.
On the next page, click “Start Check-In" at the bottom of the screen (or other appropriate boxes if patient cancelled or did not show for appointment).
Note: COVID screening questions are documented at triage. However, please ensure patient answered “no” to COVID screening questions (COVID questions are asked at hospitality).
The next page includes demographic information. All areas in yellow are mandatory (if you have questions about a field, select for explanation). Please confirm/complete as many boxes as possible (sections explained individually below in this guide):
