If you are currently without insurance and are interested in being seen in our mobile clinic, please call 865-212-5570 to schedule an appointment. Please review this NEW Patient Registration Form and, if you can print out a copy, bring it with you to your first appointment.  We also ask that you bring a photo ID and all medications (bottles) you are taking (prescription, over the counter, vitamins, supplements).
